All patients are registered with Hatfield Health Centre and are nominated a 'usual GP'. This GP is responsible for ongoing issues of your care and we will do our best to maintain continuity of care for ongoing issues. Any acute or new illnesses will be care navigated by our reception team and directed to the most appropriate clinician, which could include; community pharmacy, physio service, health and wellbeing specialist, a GP, practice pharmacist or a specialist nurse.
